The above statement is wrong and it is not mentioned exactly in same manner. In stead,the passage says, "The medical care system in the US has developed WITHOUT strong direction from the local, state, and federal governments"
How could be D is the answer and given that the statement is wrong first place?
A, Makes sense to me.

The question stem says, "Which of the following statements is NOT supported by the passage?" This is a negative question, or a question that asks for the FALSE statement from among the choices.

D is the right answer because it offers the idea that IS NOT SUPPORTED BY THE PASSAGE.
