| why would the antigens of a mother with type O blood attack the fetus with the same blood type? |

It is not because the mother and fetus have the same blood type that the mother will attack the fetus, you got that right. However the question asks what would make the mother create mostly anti-Rh antibodies.
The passage mentions that | Quote: | | the antigens of the ABO system, which are more immunogenic. | Therefore, if the fetus was of type A, B or AB the most antibodies formed would be antibodies for the A and/or B and less antibodies would form for the Rh factor.
If the fetus is of the same blood type as the mother, then only the Rh antibodies would form, making them the major antibodies formed instead of focussing most on the blood type antibodies. |
